These forms range from asking questions about current eye conditions, to medical history, and authorizations. By completing these forms in the comfort of your own home, you will be able to minimize the time you are in the office waiting to be seen by Dr. Teten. Upon arrival for your scheduled appointment, our team will help you get checked-in and taken back for all of our pretesting. Once you are complete with pretesting you can wait in our reception area, enjoy a coffee or other beverage, and checkout the available frames that we offer. Once Dr. Teten is ready for you, we will take you back to the exam room. Here Dr. Teten will perform her thorough eye exam, making note of any eye conditions that are needed to be treated along with her prescribed treatment plan. You can also always come by the office at 649 Cedar Road, Ste 101, Chesapeake, VA 23322. We are located off Cedar Road in the Great Bridge area of Chesapeake. The office building that we are located in is between the Oman Funeral Home and the Goddard School. There is ample parking located in front of the building once you get off Cedar Road. We are open Monday – Tuesday 8 PM to 3 PM and Wednesday – Friday 8 AM to 5 PM.

You may also schedule an appointment online with us. By utilizing our online scheduling you can see the available time slots that we have and pick an appointment slot that best suits your busy schedule. After your appointment is scheduled you will be prompted to fill out all of the medical history forms, which you can do at your convenience prior to your scheduled appointment.

Additionally, we have the ability for you to check out and try on virtually the frames that we have in our optical gallery. If there is a frame style that you like but wish to try it in a different color. If you have scheduled a regular comprehensive exam (non contact lens exam) appointment with us, two days prior to your appointment you will receive an email and text inviting you to virtually try on different frames we carry and make a selection of frames from which you would like to try.
